We can help you find the right Medicare Advantage plan for your needs and budget. Braven Health has options with $0 premiums, many $0 copays and low out-of-pocket costs, built-in prescription, dental, vision and hearing coverage in all of our plans.
We have PPO plans with flexible out-of-network benefit options, up to $990 per year in flexible extras, plus, even more savings and advantages!

Now, let’s take a look at our 2022 Braven Health Medicare Advantage plans.

There are three Braven Health Medicare Advantage plan options available for 2022: Braven Medicare Choice (PPO), Braven Medicare Freedom (PPO) and Braven Medicare Plus (HMO).
Each provides access to a large network that includes the entire Hackensack Meridian Health and RWJBarnabas Health systems, and many more high-quality doctors, specialists, and hospitals near you.
The PPO plans include in-network access to the Blue Medicare Advantage PPO network in 47 states and has flexible out-of-network benefits.

Who is eligible for Braven Health? There are certain eligibility requirements in order to qualify for a Braven Health Medicare Advantage plan.
First, you must have Medicare Part A and Part B. Second, you must reside in the service area for the Braven Health Medicare Advantage plan that you wish to enroll in. The service area means the county you must live in, in order to be eligible for the Braven plan. Your doctors or health care providers do not have to be located in the plan service areas.

The service area for both Braven Medicare Choice (PPO) and Braven Medicare Freedom (PPO) includes eight counties: Bergen, Essex, Hudson, Middlesex, Monmouth, Ocean, Union and Passaic.

Braven Health has a large PPO network. We have about 620,000 providers nationwide and 4,300 hospitals. Also, there are 47 states included in our out-of-state PPO network.
Slide 14 :
Braven Medicare Choice (PPO) has a zero dollar medical deductible and a zero dollar monthly premium. But that’s not all. This plan offers a $0 copay for in-network doctor visits and online doctor/therapist visits, called telehealth, through our care online program.
There are no referrals required, plus it includes routine vision, dental and hearing benefits. There are prescription copays starting at zero dollars and a zero dollar prescription deductible for Tiers 1, 2 and 6, plus only a $150 prescription deductible for Tiers 3, 4 and 5.
You get comprehensive dental coverage, multi-state network access, up to $400 annual reimbursements for Wellness Extras, up to $280 annually in over-the-counter card credits, and up to $230 annually in gift card rewards for preventive screenings. Plus, there are additional extras like in-home support services and meal delivery.

Here are the coverage details for our Braven Medicare Choice (PPO) plan for medical. As you can see, there is a zero dollar monthly premium and annual deductible, and a zero dollar copay for routine dental and telehealth for both in- and out-of network coverage.
There are zero dollar copays for in-network PCP visits and annual physical exams, which are ten dollars if you go to an out-of-network provider. There is a twenty dollar copay for an in-network specialist visit, and a thirty dollar copay for an out-of-network specialist visit.
Routine vision and hearing copays are zero dollars for in network and $30 for out of network. Inpatient hospital stays are $320 dollars for the first five days and then zero dollars after six or more days. There is worldwide emergency care coverage for a $90 dollar copay, and finally, there is a maximum out-of-pocket expense of $6,700 dollars.
Slide 16 :
Now, we will go over the prescription details of the Braven Health Medicare Choice (PPO) plan. This plan has a low deductible of $150 dollars for Tiers 3, 4 and 5, and the initial coverage begins after your deductible is met. There are six levels, or Tiers. Tier 1 is for preferred generic drugs.
With a preferred generic drug, you will have a zero dollar copay for 30-day standard retail, and zero dollar copay for mail order for 30 or 90 day. Tier 2 is generic drugs and you will pay $10 dollars for 30-day standard retail, $5 dollars for 30-day mail order or $15 dollars for 90-day mail order.
Tier 3 is preferred brand. There is a $47 dollar copay for 30-day standard retail and mail order, or a $141 dollar copay for 90-day mail order. Tier 4 is non-preferred drugs, with a $100 copay for both 30-day standard retail and mail order.
There is a $300 copay for 90-day mail order. Tier 5 is a specialty Tier and is not available for 30 or 90-day mail order. There is a 30 percent coinsurance for 30-day standard retail.
New for 2022, is Tier 6 for select care drugs, which includes generic drugs that support conditions related to diabetes, hypertension, osteoporosis and high cholesterol. For Tier 6, you have a zero dollar copay for all options.
Slide 17 :
Next, we will talk about Braven Medicare Freedom (PPO). This is another zero dollar medical deductible and no cost for in-network doctor visits. With this plan, you have a $35 Premium, a zero dollar copay for online doctor and therapist visits, and there are no referrals required.
Plus it includes routine vision, dental and hearing benefits. Prescription copays start at zero dollars and there is a zero dollar prescription deductible for Tiers 1, 2 and 6, plus only $100 for Tiers 3, 4, and 5. There is comprehensive dental coverage and multi-state network access.
You get up to $400 in annual reimbursements for Wellness Extras, up to $360 annually in over-the-counter card credits and up to $230 annually in gift card rewards for preventive screenings. Plus, there are additional extras like in-home support services and meal delivery.

Here are the coverage details for our Braven Medicare Freedom (PPO) plan for medical. As you can see, there is a zero dollar annual deductible and a $35 monthly premium. There is a zero dollar copay for in-network PCP visits, annual physical exams and routine vision and hearing visits, and you pay 30% coinsurance for out of network.
There is a $20 copay for in-network specialist visits and 30 percent coinsurance for out of network. Routine dental and telehealth is zero dollars for both in and out of network. Inpatient hospital stays at an in-network provider are $300 dollars for the first five days and then zero dollars after six or more days.
Out of network has 30 percent coinsurance. Worldwide emergency care is a $90 dollar copay, and finally, there is a maximum out-of-pocket expense of $6,500 dollars.

Now, we will go over the prescription details of the Braven Health Medicare Freedom (PPO) plan. This plan has a lower deductible of $100 dollars for Tiers 3, 4 and 5. Initial coverage begins after your deductible is met.
All other costs are identical to the Braven Health Medicare Choice (PPO) plan. Tier 1, or preferred generic drugs, has a zero dollar copay for standard retail and mail order. Tier 2, or generic drugs, has a $10 dollar copay for 30-day standard retail, $5 dollar copay for 30-day mail order or $15 dollar copay for 90-day mail order.
Tier 3 is preferred brand, with a $47 dollar copay for 30-day standard retail and 30-day mail order, or a $141 dollar copay for 90-day mail order. Tier 4, or non-preferred drugs, has a $100 copay for both 30-day standard retail and 30-day mail order. There is a $300 copay for 90-day mail order.
Tier 5, specialty Tier, is not available for either 30 or 90-day mail order. There is 30 percent coinsurance for 30-day standard retail. Tier 6 is new for 2022, which is for select care drugs, including generic drugs that support conditions related to diabetes, hypertension, osteoporosis and high cholesterol. For Tier 6, you have a zero dollar copay for all options.

The service area for Braven Medicare Plus (HMO) is in 7 counties, including Bergen, Essex, Hudson, Middlesex, Monmouth, Ocean and Union. Unlike our PPO plans, Passaic County is not included for the Braven Medicare Plus (HMO) plan.
With this plan, you must use physicians, health care professionals and facilities that participate in the Braven Medicare Plus (HMO) plan. The network extends throughout New Jersey.
Slide 21 :
With Braven Medicare Plus (HMO), there is a zero dollar monthly premium, medical deductible and prescription deductible. Benefits include a $0 copay for Primary Care Physician (PCP) visits and zero dollar copay for online doctor or therapist visits through our care online program.
There are no referrals required, plus you have routine vision, dental and hearing benefits. Prescription copays start at $0 and you get up to $400 in annual reimbursements for Wellness Extras, up to $280 annually in over-the-counter card credits, and up to $230 annually in gift card rewards for preventive screenings.
This plan also has comprehensive dental coverage, plus additional extras like in-home support services and meal delivery.

Here are the coverage details for our Braven Medicare Plus (HMO) plan for medical. For this plan, you must use a participating provider in the Braven Medicare Plus (HMO) Network. Please visit our online directory at BravenHealth.com to see if your doctor participates in this plan.
As you can see, there is a zero dollar monthly premium and annual deductible. There is a zero dollar copay for PCP visits, routine vision, hearing, dental and telehealth. There is a twenty dollar copay for specialist visits. Inpatient hospital stays are $325 dollars for the first five days and then zero dollars after six or more days.
There is a $195 copay for ambulatory surgical centers, $90 copay for worldwide emergency care and a maximum out-of-pocket expense of $6,500 dollars.

Now, we will go over the prescription details of the Braven Medicare Plus (HMO) plan. This plan has a zero dollar deductible. Tier 1, or preferred generic drugs, has a zero dollar copay for standard retail and mail order.
Tier 2, or generic drugs, has a $10 dollar copay for 30-day standard retail, $5 dollar copay for 30-day mail order or $15 dollar copay for 90-day mail order. Next, we will go over more benefit highlights.

First, regardless of your plan, your annual physical exam is covered with a zero copay with in-network providers. The exams include detailed medical and family history, a head-to-toe assessment and includes one visit each year.
Slide 26 :
Next, dental benefits are included with a zero copay for routine dental services. Covered benefits include three oral exams per year, three cleanings per year, one fluoride treatment every six months and one full mouth X-ray every three years.
For the PPO plans, out-of-network coverage is available at the same cost share. You will receive the best value by seeing a participating provider. A list of participating providers can be found on our website at BravenHealth.com.
Slide 27 :
Our plans offer comprehensive dental coverage, which includes 50% coinsurance up to $1,000 maximum coverage amount every year for restorative, endodontics, periodontics, and simple extractions.
This excludes crowns, onlays, prosthodontics, implants, orthodontia, other oral or maxillofacial surgery, dentures, inlays, sealants, space maintainers and missing teeth. The coverage maximum does not apply to preventive and diagnostic services.
Slide 28 :
For hearing benefits, the covered in-network benefits include a zero dollar copay for routine hearing exams, a zero dollar copay for hearing aid evaluation and fitting, and you get up to $1,250 annually for hearing aids.
For Braven Medicare Plus (HMO), routine hearing exams and hearing aid services must be coordinated through HearUSA. Braven PPO members can go to in- or out-of-network providers for hearing aids. Also, PPO members pay out of pocket and submit reimbursement forms.
Slide 29 :
For vision benefits, the covered in-network benefits include a zero dollar copay for routine annual eye exams, a zero dollar copay for eyeglasses or contact lenses after cataract surgery and up to $200 every two years for eyewear or eyeglasses not associated with cataract surgery.

Now for the extra savings and advantages with Braven Health.

The OTC Benefit card gives you credits every three months - which is at least $70 dollars - to purchase over-the-counter personal health items online or from participating retailers like Walmart, CVS, Walgreens and more.

With our annual flex benefit reimbursement, you receive up to $200 every year for any combination of WW, acupuncture, nutritional therapy, an activity tracker, bathroom safety devices and therapeutic massage. And, there are additional hours for in-home support services with Papa.
Slide 33 :
The fitness reimbursement allows for a $200 annual reimbursement towards gym memberships, including a yoga studio, virtual at-home fitness programs, and fitness equipment, including hand-held free weights, exercise bands or yoga mats.
Slide 34 :
Our next list of benefits give you more ways to get the care you need – right from home. The first is the home delivered meal benefit. This benefit includes meals after an acute inpatient hospital stay or rehabilitation admission.
You may be eligible to receive up to 28 nutritious meals over a 14-day period to help you recover from your illness. Delivery of meals is included. This meal benefit is limited to one occurrence per calendar year.

Next, is our telehealth benefit. This includes unlimited online doctor and therapist visits using your phone, tablet or computer – all for $0 with our care online program.
Slide 36 :
Our 24-hour nurse line gives you immediate answers to your health questions. Anytime, from anywhere, 24 hours a day, seven days a week. With our nurse line, you have access to caring, registered nurses who have an average of 15 years of clinical experience.
Slide 37 :
In-home support services with Papa. This benefit is new to Braven Health members for 2022. You will get support services provided right to your home, even virtually. These services include transportation for grocery shopping, medication pick up, and doctor's appointments.
You can also get technical guidance, reminders, light house help, light exercise and more! There is a zero dollar copay for up to 24 hours per day. Additional hours can be purchased using your Flex Benefit allowance.
Slide 38 :
Prescription Delivery. There are three convenient ways to get your prescriptions delivered to you. PillPack by Amazon Pharmacy allows you to get your prescriptions, over-the-counter medications and vitamins packaged, according to your doctor’s instruction, into individual packets by the times of day you take them.
Express Scripts® Pharmacy, which is new to Braven Health members for 2022, offers safe, accurate prescriptions delivered to your door. You get free standard delivery in weather-proof packaging.
AllianceRx Walgreens Prime gives you up to a 90-day supply of your medicine delivered right to you, anywhere in the United States.
Slide 39 :
We also offer benefits for members with Chronic Conditions. There is a nutritional food allowance for eligible members with a qualifying chronic illnesses, such as cardiovascular disorders, chronic heart failure and diabetes. There is a $75 per quarter allowance, which is every three months, to use on eligible grocery items at participating retailers.
Eligible members can also use their $75 per quarter allowance to have a choice of fresh fruit and produce boxes delivered right to their home from FarmboxRx.
Our Part D Senior Savings Model allows members to pay only $35 for a monthly supply of covered Insulins during deductible, initial and coverage gap phases.
