Care Coordination and Why It’s Important

If you have multiple health conditions or complex health needs, such as a mental health disorder, your outcomes can improve if your care providers work together. This is called care coordination. Care providers may be a Primary Care Physician, home health aide, or a caregiver or relative involved in your treatment.

Personal Representative

A Personal Representative Can Help With Your Health Care

It can be hard to take care of your health, especially if you’ve been diagnosed with a behavioral health disorder or a Substance Use Disorder (SUD). If you live in New Jersey, you can get help managing your health, if you need it.

You can ask for a limited personal representative or a legal personal representative to help you manage your health care.

If you choose a personal representative, you do not lose any of your rights. You can always choose to handle your own benefits at any time. Your personal representative can be your husband, wife, son, daughter, relative, friend or a court-appointed individual.

Here are some of the differences between a limited personal representative and a legal personal representative.

Limited Personal Representative

A limited personal representative is someone you choose to have access to some of your personal health information from Braven Health, including the name of your condition, the type of treatment you’re getting, the name of your doctor and financial information.

With your permission, your limited personal representative can access more sensitive information about you, like whether you have:

  • HIV or AIDS
  • A sexually transmitted disease
  • A mental health condition or SUD

While your representative may have access to your information, they will not be able to make changes to your health care coverage.

Legal Personal Representative

A legal personal representative is someone you allow to have full access to all of your health information, including your sensitive information.

A legal personal representative can also make changes to your health care coverage and get your health care-related mail.

Appointing a Personal Representative

To appoint a personal representative, download a personal representative form. They are available in English and Spanish. You can submit completed forms by mail or fax:
