Vision Benefit Highlights
Braven Health gives you vision benefits to keep your eyes healthy. In 2024, we're partnering with Davis Vision to help provide some of your routine care.
New for 2024
In 2024, Davis Vision provides Braven Medicare Choice (PPO) and Braven Medicare Freedom (PPO) plan members with vision benefits for routine eye care.
When you use Davis Vision network optometrists and eyewear retailers, you pay:
- $0 copay for basic eyeglass lenses
- $0 copay for one routine eye exam per year with an optometrist
These glasses include:
- One pair of clear, plastic lenses in any prescription, including:
- Single vision
- Bifocal
- Trifocal
- Lenticular
- Standard progressive
- Tinting of covered plastic lenses, either solid or gradient
- Scratch-resistant coating of covered plastic lenses
Note: Certain Davis Vision providers give discounts on other lens upgrades to Braven Health members. You’ll have to pay out-of-pocket for these upgrades. To learn more, call Davis Vision at 1-888-257-1267 (TTY 711).
When you go out of network, you pay:
- 50% of the cost for eyeglass lenses
- 50% of the cost for a routine eye exam
You also have a $150 allowance every year for prescription eyeglass frames or contact lenses from in- or out-of-network providers. You'll get the most out of your benefit by using Davis Vision’s network of eyewear retailers. You must have a valid prescription to get eyewear. Non-prescription polarized sunglasses aren't eligible for this benefit.
Note: Braven Medicare Group members don't get routine eye benefits from Davis Vision. Take a look at your 2024 Evidence of Coverage for more information about your vision benefits.
What Is a Routine Eye Exam?
Any exam related to an eyeglass or contact lens prescription is routine. These might include a series of tests to check your vision and the health of your eyes, and to update your prescriptions.
During a routine exam, your optometrist will check your vision and screen for certain conditions like near-sightedness, far-sightedness or astigmatism. They’ll also write a prescription for eyeglasses or contact lenses if you need them.
Davis Vision has a network of optometrists and eyewear retailers. You can find them throughout New Jersey and at in-store eyewear retailers like Walmart and Visionworks. Just show them your Braven Health member ID card and your eyewear prescription to get started.
To find Davis Vision providers, you can:
- Call Davis Vision at 1-888-257-1267 (TTY 711) (Monday – Friday: 8 a.m. to 11 p.m., Saturday: 9 a.m. to 4 p.m., Sunday: noon to 4 p.m., ET).
- Register for an account at DavisVision.com. Click on Member Log In, and click Register new account. In addition to providing your full name and date of birth, you must provide your Braven Health member ID number and email address. Enter your member ID number beginning with 3HZN.
- Visit BravenHealth.com and click Menu > For Members > Extra Benefits > Vision Benefits. You'll find a downloadable, printable directory of Davis Vision network optometrists and eyewear retailers.
Using Out-of-Network Providers & Retailers
If you visit a provider outside of Davis Vision’s network, you’ll pay 100% of the cost upfront. You must then ask Davis Vision to reimburse you 50% of the cost of:
- Your annual routine eye exam
- Eyeglass lenses
For eyeglass frames or contact lenses: you’ll be reimbursed for amounts you paid out-of-pocket up to your $150 annual allowance.
To request reimbursement, print and fill out the Vision Services Reimbursement Form, and mail it to the address listed on the form.
When You Need More Than Routine Eye Care
Davis Vision covers your annual routine eye exam and routine eyewear only. When you need to see an ophthalmologist or other doctor for exams to treat a medical condition that affects your vision, or if you need a diabetic eye exam, glaucoma screening or eyewear after cataract surgery, you can find a provider at BravenHealth.com/find-doctor. These services are covered by Braven Health both in- and out-of-network.
You’ll have a $0 copay for:
- Eyeglasses or contact lenses after cataract surgery
- Diabetic retinal exams for people with diabetes. During this exam, your provider will check for changes in the blood vessels in the retina, the part of the eye that senses light.
You'll pay a copay for exams to diagnose and treat diseases and conditions of the eye. Check the Vision care section of the Medical Benefits Chart in Chapter 4 of your Evidence of Coverage and look for the copayment amount for an in-network diagnostic eye exam.
