Rewards for Wellness

As a Braven Health member, you have access to a wellness program that rewards you for staying on top of your health checkups. With Healthy Journey Rewards, you can earn money to use at participating stores.

In 2024, you could get up to $350 for these routine health screenings:

• Annual wellness visit - $75
• Breast cancer screening - $50
• Bone mass density testing - $50
• Colorectal cancer screening - $50
• Diabetes management:
  • Diabetic HbA1c test - $25
  • Diabetic retinal eye exam - $25
  • Diabetic kidney function test - $25
  • Diabetic self-management training (classes to control your blood sugar) - $25
• Healthy heart self-management training (classes to control your blood pressure) - $25

The dollars you earn can be used at:

• Chipotle
• Dunkin’
• Home Goods
• iTunes or Apple Store
• Macy’s
• Marshalls
• Panera Bread
• PetSmart
• Starbucks
• Subway

You can get your rewards one of these easy ways:

• Log in to BravenSmartCard.com to complete and submit a Health Screening Attestation form in the Rewards & Incentives section.

• You can also call Braven Health Smart Card Services at 1-800-688-9140 (TTY 711), weekdays, 8 a.m. to 8 p.m., ET. (You’ll need to provide the name of the completed screening/training, where you received the screening/training, and the date you completed the screening/training.)

Once confirmed, your rewards will be automatically loaded onto your Smart Card. You can go back to BravenSmartCard.com to see your rewards balance at any time.

If Your Smart Card’s Not Active Yet

You need your Braven Health Smart Card to redeem your benefits in the Healthy Journey Rewards program. Follow these steps to get started:

1. Activate your card. Call 1-800-688-9140 (TTY 711) or visit BravenSmartCard.com. You have to activate it before you can use it.


2. Once it’s activated, you can use your card for any of the Healthy Journey Rewards you’ve earned.


3. Use your card to redeem rewards at participating vendors. The full list is at BravenSmartCard.com, or you can call 1-800-688-9140 (TTY 711).


4. Download the MyTotalBenefits app from the app store on your smartphone. Use it to:
   • Look at your balances
   • Scan item bar codes to check items’ eligibility
   • Buy items
   • Manage your account

   Keep in mind your reward funds will be available as long as you're a Braven Health member.
